The vent screen located on the well cap is torn. Please repair/replace the screen to deter the
entry of pests.
2. During the inspection several bolts on the well cap were loose. Please ensure that all bolts
are tight to ensure a good seal to prevent contaminants from entering the well.
3. Your cross connection control program was approved by this office on September 20, 2004.
Be reminded, as part of that program, to inspect your distribution system at least once
annually and install cross connection or backflow prevention devices as needed. At a
minimum, atmospheric vacuum breakers must be installed on all threaded hose bibs that
don't have built-in anti-siphon capability. If you have any "testable" devices, they must be
inspected and tagged every year by a certified technician. You are required to keep records
of your inspections for ten years and have them available for review upon request.
IRGINIA
VDHODvF H ALTHNT
Protecting You and Your Environment
