N
DEC 012014
COMMONWEALTH of VIRGINIA
Marissa J.Levine,MD,MPH,FAAFP DEPARTMENT OF HEALTH 400 S.Main Street,2nd Floor
State Health Commissioner OFFICE OF DRINKING WATER Culpeper,VA 22701
John J.Aulbach II,PE Phone:540-829-7340
Director,Office of Drinking Water Culpeper Field Office Fax:540-829-7337
NOV 2 4 2014
Groundwater System Sanitary Survey Report
To: Mr. Mark Whaley SUBJECT: Loudoun County
47001 Fairway Drive WATER: Algonkian Park—Pool
Sterling,VA 20165 PWSID No.: 6107016
Survey Date: 10-28-2014
Present at Survey: Mark Whaley, Chris Wright and David Pinner
Next Survey Scheduled For: September 2017
Future Sampling Requirements:
Quarterly Entry Point Combined Nitrate/Nitrite Sample Due: 3rd Quarter 2015
Quarterly Distribution Routine Bacteriological Sample Due: 2"d Quarter 2015
As a result of the sanitary survey noted above, the Department offers the following comments.
Should you have questions or desire to discuss our findings, or want a copy of the inspector's field
evaluation notes, please contact us at 540-829-7340.
1. The double check valve located after the pressure tanks is leaking. Replace or repair the check
valve as necessary. Please note that a check valve to prevent backflow into the well is
recommended on the well discharge line prior to the pressure tanks. However, the valve does
not need to be a testable device, such as a double check valve.
2. The pump house floor below the leaky double check valve is rotting and shows recent pest
activity. Please repair the check valve as mentioned in comment 41 and repair the floor to
prevent pests from entering the pump house.
3. Your cross connection control program was approved by this office on September 20, 2004. Be
reminded, as part of that program, to inspect your distribution system at least once annually and
install cross connection or backflow prevention devices as needed. At a minimum, atmospheric
vacuum breakers must be installed on all threaded hose bibs that don't have built-in anti-siphon
capability. If you have any "testable"devices, they must be inspected and tagged every year by
a certified technician. You are required to keep records of your inspections for ten years and
have them available for review upon request.
vIRGINIA
VDHODF EALTHNT
Protecting You and Your Environment
